Summary of Position

This position is responsible for providing patients with assistance in meeting their healthcare needs including enrollment in Medicaid Expansion application assistance, pharmaceutical charity programs, Medicare, ACA Health Insurance, preparing disability applications for patients through the SOAR program, providing education and completing paperwork for sliding scale and new uninsured patients, Medicaid Expansion application assistance, and referring patients to other community resources based on their expressed needs. Address all patient referrals through the Provider Referral Work Que. Coordinate outreach/in reach events during National Health Center Week and beyond, create and maintain relationships with community agencies and elected officials. Equitably serve the organization’s patient population through patient advocacy, community, outreach, and engagement.

Minimum Qualifications

However, any equivalent combination of experience, education, and training which provides the necessary knowledge, skills, and abilities would be acceptable, subject to any legal and/or regulatory requirements; Must be able to sit, stand and walk for long periods of time; Ability to read and understand the English language; Ability to effectively maintain confidentiality of records and communicate with all levels of personnel.

  • Experience : Experience in a medical office setting required.
  • Bilingual (English/Spanish) : Preferred but not required.

  • Additional Required Skills : Knowledge of medical office software and comfort in using computers in daily work activity. Must have excellent verbal communication skills. Excellent organizational skills. Must be able to problem solve and have critical thinking skills. Understand and maintain patient confidentiality.
  • Additional Requirements : Maintain active membership status in the respective Continuum of Care Program and possess the ability to plan and facilitate community outreach events.
  • Additional Preferred Skills: Basic medical terminology. Proficient in use of all computer software utilized in the organization, including Epic, NC Tracks, Marketplace Assister Community (MAC), ACA software, Connector utilization, SHIIP software, The Pharmacy Connection, and Microsoft Office Products.

Education: High School Diploma or GED certificate required; bachelor’s degree preferred

Certification(s): SHIIP Counselor, Affordable Care Act (navigator or certified application counselor), SOAR Counselor, ACA Navigator, completion of TPC training, Community Health Worker Certification

K ey Responsibilities

  1. Provide excellent customer service to all constituencies – community members, patients, fellow employees, community agency staff.
  2. Utilizing motivational interviewing techniques, ascertain individuals’ current needs and resources.
  3. Based on information provided by the individual served, provide education about requirements for programs as well as program enrollment and/or referral.
  4. In both internal and external interactions, uphold Medicaid Expansion, MAC, MAP, Medicare, disability, ACA insurance, FQHC, and community agency enrollment and program guidelines, policies, and procedures.
  5. Participate in QI initiatives to improve processes and minimize errors and variation.
  6. Collaborate well with all appropriate staff to maintain efficient workflow.
  7. Utilize data systems to update and communicate with staff to receive appropriate referrals and expedite patient processing.
  8. Adhere to all reporting, training, and certification requirements of Community Health Worker Level – 1, Medicaid Expansion, MAC, SOAR, SHIIP, ACA, FQHC, and other community programs.
  9. Recruit additional volunteers to increase capacity to provide SHIIP and SOAR services.
  10. Document activities in Marketplace Assister Community (MAC), The Connector, TPC and other means as requested by team leads and/or program manager.
  11. Maintain positive relationships with community members, healthcare providers and community partners.
  12. Develop and maintain positive relationships with community members and agencies through community outreach and engagement.
  13. Physical demands may include transportation of event supplies, set up and breakdown of tables, chairs, booths, and IT equipment at enrollment events.
  14. Perform other duties as assigned.
